Join us as we explore the new Chronic Conditions Management (CCM) framework—designed to simplify care planning, strengthen team-based collaboration, and improve outcomes for patients living with chronic illness. This event will unpack the key reforms, including the new GP Chronic Condition Management Plan (GPCCMP), MyMedicare-linked access, and updated referral pathways for allied health.

The workshop covers the RACGP standards on triage, identifying high-risk patients, implementing practice policies and systems, techniques of triage and medico-legal issues.

Talking About Suicide in Practice will equip participants with the skills and confidence in recognising high risk of suicide in patients along with undertaking a detailed risk assessment.  Participants will also learn management planning using a collaborative-team approach.
