From 1 July 2025, Australia’s approach to chronic condition care is evolving. Join us as we explore the new Chronic Conditions Management (CCM) framework—designed to simplify care planning, strengthen team-based collaboration, and improve outcomes for patients living with chronic illness. This event will unpack the key reforms, including the new GP Chronic Condition Management Plan (GPCCMP), MyMedicare-linked access, and updated referral pathways for allied health.

• Describe the role of MyMedicare in improving access to care for individuals with chronic conditions and explain how it supports continuity of care.
