We would like to invite GPs, practice nurses, and practice staff to our upcoming Primary Sense Lunch & Learn Series.

By the end of this session, participants will be able to:

1.Describe the purpose and methodology of ACG Complexity Scores, including how they are calculated and used to assess patient complexity in primary care.

2.Explain how Complexity ACG Scores can support proactive patient care, including identifying patients who may benefit from enhanced care coordination, chronic disease management, and targeted outreach.

3.Apply Primary Sense complexity insights to population health management, using real-world examples to prioritise care planning, drive quality improvement initiatives, and improve patient outcomes.
