This full day of CPD education aims to empower health professionals to support patients to live well for longer in the community.

• Prevention of cognitive decline, post diagnostic and ongoing management of dementia 
• Addressing what is important to older adults, social prescribing and intergenerational programs
• Chronic disease management including diabetes care, nutrition, falls prevention, polypharmacy and deprescribing
