We invite Credentialled Diabetes Educators, Accredited Practising Dietitians or organisations employing these professionals to attend an information session about our upcoming opportunity to pilot a multidisciplinary team approach to managing type 2 diabetes in general practices in eastern Penrith.
This 45-minute information session will provide a high-level overview of the project and its purpose, including details about the co-design process, requirements and expectations. It will also be an opportunity to understand the application process (including eligibility) and ask any questions before the EOI opens on 16 October 2024.
